To receive a certificate, you need to declare your candidacy before you complete the coursework. Many programs have unique certificate candidacy forms that you can find on each program's web page. However, here is the standard Certificate Candidacy form.

Note: In most cases it is not necessary to enroll in a Certificate Program in order to take individual classes.
